Autism Spectrum Disorder (ASD) Evaluation
$2,000 flat fee ($2,250 if paid in 3 installments)
1st Appointment Intake Interview
At the Intake (60-90 minutes), Dr. Khalil will interview the caregiver(s) and the identified patient and determine if further autism testing is needed or can be ruled out. This can be done virtually if needed.
If autism is ruled out, no further testing is needed, and a flat rate of $300 is due; no more is charged, and no further appointments occur.
2nd Appointment Testing—Dr. Khalil will have an informant, typically a caregiver, complete mental health, behavioral, autism-specific measures, and adaptive rating forms. The patient will be assessed by Dr. Khalil using gold-standard autism-specific measures (e.g., ADOS-2) and cognitive or developmental testing as appropriate. The patient may also be asked to fill out rating forms, depending on their age and presentation. This must be in person for children younger than 36 months.
3rd Final Appointment- Feedback (45-60 minutes)
Dr. Khalil will take approximately 4 weeks to score the measures and write a report, including recommendations, test results, and a summary for future reference. Dr. Khalil will review the results and diagnoses with the family and answer any questions they may have. Dr. Khalil will provide recommendations for at-home and school use that may be helpful based on the testing results. This can also be done virtually if needed.
ADHD Testing
$1,500 flat fee ($1,750 if paid in three installments)
Includes a 60-90 minute clinical interview, cognitive testing, adaptive behavior measures, and attention and executive functioning measures from caregivers and teachers. Trauma and emotional screening is included when appropriate.
A reduced fee of $500 may be available if recent cognitive testing is available.
Cognitive/Intellectual Disability Testing
$1,500 flat fee ($1,750 if paid in three installments)
Includes a full cognitive assessment, adaptive functioning measures, and a 60–90-minute caregiver interview. Evaluations are designed to clarify developmental and learning needs and may include screening for autism, attention concerns, and other mental health concerns as needed. Appropriate for families seeking diagnostic clarity or documentation for services or accommodations. However, if autism testing is required, the total cost will be based on the autism rate. Before making any testing changes, Dr. Khalil will consult with the patient and their caregivers.

First Steps Evaluations (No Cost to Families)
Dr. Khalil is a First Steps-enrolled psychologist and provides autism and developmental evaluations for children under age 3 through Indiana’s early intervention program. If your child is receiving services through First Steps, evaluations are provided at no cost to your family.
Assessments are conducted in-home or at a community location in Allen County and include a virtual intake and feedback appointment. Telehealth evaluations for First Steps are offered to counties surrounding Allen. Contact your First Steps coordinator to send a referral.
If you have out-of-network benefits through your insurance, you may be eligible for partial reimbursement for psychological testing services. Here’s how the process works:
1. Pay for Services Upfront
You’ll pay the full testing fee directly to our practice at the time of service. We do not bill insurance directly but can provide the documentation needed for reimbursement.
2. Receive a Superbill
After services are completed, you’ll receive a detailed superbill—an itemized receipt that includes all necessary information required by insurance, such as:
Provider name, credentials, and NPI number
Diagnosis code(s) (if applicable)
CPT codes for each service
Dates of service and session fees
3. Submit the Superbill to Your Insurance
You can submit the superbill through your insurance provider’s online portal, by mail, or by fax—depending on their process. We recommend calling your insurance company first to ask:
Do I have out-of-network benefits for psychological testing?
What percentage of the cost is reimbursed?
Do I need a referral or prior authorization?
Where and how do I submit a superbill?
4. Receive Reimbursement
If your plan covers out-of-network testing, your insurance company will reimburse you directly for a portion of the costs based on their policies.
